Most humans and most animals sleep on their side. It turns out that may not be a coincidence.
A patient asked me last week whether the way she sleeps could be doing anything for her brain. She had read something online and wanted to know if it was real. Here is what the research actually shows. The details matter.
Your brain has a waste clearance system called the glymphatic pathway. It runs mostly during deep sleep and flushes out metabolic byproducts, including a protein called beta-amyloid, that build up while you are awake.
Beta-amyloid is the same protein that accumulates in Alzheimer's disease. That does not mean poor sleep causes Alzheimer's.
It does mean the brain relies on sleep to clean itself, and disruptions to that process are one of many factors researchers are studying in neurodegeneration.
A 2015 study by Lee and colleagues, published in the Journal of Neuroscience out of Stony Brook University, used dynamic-contrast MRI to measure glymphatic clearance in rodents in three positions. Lateral (side) sleeping produced the most efficient waste clearance compared with sleeping on the back or stomach.
The rodent finding has not been directly replicated in humans. But a 2024 study in the Proceedings of the National Academy of Sciences confirmed that the glymphatic system functions in the human brain during sleep.
So we have a plausible mechanism, a strong rodent study, and growing human data. Not proof, but a reasonable working hypothesis.
Weird, right? A sleep position may matter more than researchers thought.
Now here is the piece that matters for women in perimenopause and beyond. NREM sleep, the deep stage when glymphatic clearance is most active, decreases with age.
Estrogen also shapes sleep architecture, and its decline is associated with lighter, more fragmented sleep. That means the very sleep that does the brain-clearing job is harder to get in the years you might most want to protect it.
You wake up foggy after what looked like eight solid hours in bed. Sound like your morning?
Some of that is fragmentation. Some of it is the shortening of deep sleep stages. The clearance system does not run as long as it once did, and that is not your fault.
The reframe is not that something is wrong with you. Your sleep is different because your brain is running a different program. If you already sleep on your side, keep doing it. There is a reasonable case that your brain likes it.
If you sleep on your back, you do not need to force yourself into a different position. Fragmentation and total deep-sleep time matter more than posture, based on what we currently know.
The stronger practical step is protecting your deep sleep. Keep your bedroom cool, around 65Β°F or 18Β°C, because core body temperature drops during NREM and a cool room supports that shift.
Limit alcohol within three hours of bed. Even one glass reduces deep sleep and disrupts glymphatic clearance, and the effect tends to get more noticeable after 45.
One note. If you snore heavily, wake gasping, or feel unrefreshed no matter how long you slept, ask your doctor about a sleep apnea screening.
Untreated apnea is one of the clearest and most treatable disruptors of the clearance system we know about.
Your brain after 45 is not failing you. It is asking for better sleep, and better sleep is something you can build.
Some nights will still be broken. Nobody sleeps perfectly at any age.
